The Steps of the IVF Treatment

After the first evaluation of your doctor, you must see your doctor once more. During this last visit, you will talk about your treatment plan. During these visits, your uterus can be measured by your doctor using a catheter. This catheter is a thin and flexible tool which is used to placing the embryo into the uterus. The catheter is inserted thru cervix and the uterus, which allows your doctor to measure the shape and dimensions of your uterus. This process provides great information for placing the embryo in the uterus. Also, some blood tests will be performed in order to find any kind of infections you or your spouse might have. A sperm specimen is harvested from men for analysis. If there are any unusual findings in these tests, another sample might be needed for further tests. The nurses also inform the patient about the treatment plan and how to use medications.

Stimulating the Eggs:

The IVF treatment starts with stimulating the eggs 1 week before the menstrual cycle. In this period, GnRH anaoluge drugs are used for stimulation. These drugs substantially reduce the release of hormones which controls developing the eggs. After menstruation, the effect of the drugs on the development of the egg will be checked using vaginal ultrasound and blood tests. If it is not decreasing, the drug treatment might continue for 1-2 weeks more.

When the function of the ovaries temporarily stops, the real stimulation treatment begins. In order to stimulate the follicles on the ovaries, hormone treatment will be applied for 10 days. For following the growth of the follicles, ultrasound and blood tests will be applied during the treatment. The use of medicine continues for 10 – 12 days. When the eggs are stimulated enough, hCG (which is another hormone) will be injected. After 36-38 hours of the hCG injection, the eggs will be harvested.

Harvesting and Fertilization of the Eggs

The harvesting process will be done under anesthesia. It usually takes for 20 – 50 minutes. The fluid which is taken from the follicles will be immediately sent to the embryology laboratory. In the laboratory, the eggs will be inserted into a solution and placed into the incubator. The time eggs spend inside the incubator depends on their maturity. On the egg harvesting day, the sperm specimen will be provided by masturbating. Under special circumstances, this specimen can also be obtained outside of the medical center. Especially, men who have difficulties masturbating in a place other than the home can provide the sample at their home. The sperm sample must be handed out to the medical center in an hour. If the time elapsed between providing and handing out the sperm might be over an hour, the process must be done inside the medical center rather than a safe location. If the sample will be brought to the medical center from a distant location, it must be carried in room temperature, without exposure to light and too hot or too cold temperatures. Rarely, a second specimen might be needed on the same day. The next process is the meeting of the sperm and egg. The fertilization ratio of the egg is usually 80%. The eggs will be under observation for 24 hours after the harvest. During this time, the fertilization process will occur. If it is necessary, some of the embryos could be contained by freezing them.  If the pregnancy does not occur after the treatment, frozen embryos can be used again. In this way, the patient does not need to repeat the egg stimulation process again. After the process, the patient will be discharged in 2-4 hours and rests for 24 hours at home. After 1-2 days of the egg harvesting process, pelvic pain (similar to menstrual pain) or blood smudges (red or brownish) might occur. This is natural, and it does not affect the pregnancy chance.

If any of the following situations occur, you may need to contact your doctor:

  • Body temperature measured over 38 degrees Celcius.
  • Excessive vaginal bleeding ( Minimal bleeding is natural)
  • Sharp and continuous pain (Minimal pain is natural)
  • Weight gain more than 1 kilos in consecutive 2 days.
  • Not being able to urinate with increasing pain in 8-10 hours after the process

Embryo Transfer

After harvesting the eggs, embryo transfer can be done in 1 to 3 days. The amount of transferred embryos depends on the quality of the embryo, but the legal limit in Turkey is 3. After the embryo transfer, there must be a 2-day rest period. 12 day after the transfer, a Blood Pregnancy Test ( presence of beta hCG on the blood) will be done. Even with an event of menstrual bleeding, the pregnancy test must be done because the bleeding might be vaginal bleeding caused by the embryo’s placement in the uterus. If the test is positive, an ultrasound will be performed 3 weeks after the test.
